I am an amateur under the UK:A Athletics and the Welsh Athletics, and I will be of the appropriate age for the race I have chosen on the day of the race. I accept that the organisers will not be responsible for any loss, damage, action, claim, costs or expenses which arise in consequence of participation in this event, howsoever caused. I confirm that I have no cause or disability that needs to be brought to your attention that will affect my ability to participate. I further declare that you may use my name, voice or picture in any broadcast, telecast, promotion or other account. I consent to the use of my name or records pursuant to the Data Protection Act 1998.
